The reason is one blood marker that almost nobody on a standard executive medical has ever been tested for: lipoprotein(a), or Lp(a). For men over 40, this is the single most under-tested number in your cardiovascular profile — and as of the 2026 ACC/AHA dyslipidemia guideline, that's officially no longer acceptable.

What Lp(a) Actually Is — And Why Your Panel Doesn't Show It

Lp(a) is an LDL-like particle with a sticky extra protein, apolipoprotein(a), bolted onto it. That extra protein makes it more inflammatory, more thrombogenic, and far more atherogenic per particle than ordinary LDL. Think of it as LDL with a barbed hook attached — it doesn't just deposit cholesterol, it actively promotes clotting and arterial damage.

Critically, Lp(a) is almost entirely genetic. Your level was set by the age of five and barely moves with diet, exercise, or weight loss. Around one in five men over 40 carries an elevated level. Most of them have no idea, because the standard lipid panel ordered at an annual physical does not include it.

If you've ever read about a fit, lean executive in his late forties who dropped from a heart attack with "no risk factors," there's a strong chance high Lp(a) was sitting silently in the background.

Why Lp(a) Matters More for Men Over 40

By the time you're in your forties, your cardiovascular system has spent two decades being exposed to whatever lifetime burden of Lp(a) you inherited. The damage is cumulative. Modelling work from the lipid community suggests that elevated Lp(a) effectively adds the equivalent of 10–15 years of arterial ageing to your risk.

The Genetic Lock-In

Because Lp(a) is genetic, knowing your number doesn't just protect you — it protects your children. If your Lp(a) is high, your first-degree relatives (sons, daughters, siblings, parents) have a meaningful probability of carrying the same risk. The 2026 guidelines explicitly recommend cascade testing of family members once a high Lp(a) is identified in one member.

The Interaction With ApoB and CAC

Lp(a) doesn't operate in isolation. Its risk compounds with every other cardiovascular variable you have. Elevated ApoB on top of elevated Lp(a) is not additive — it is multiplicative. A high CAC score plus high Lp(a) is a five-alarm signal. This is why the executive cardio triad — ApoB, CAC, Lp(a) — is the only blood-and-imaging combination that actually gives you a real risk picture by your forties.

When to Test Lp(a) — And How Often

The answer for every man over 40 is the same: once, now, and that's it.

Lp(a) is so stable across a lifetime that a single measurement is enough for the vast majority of people. The 2026 ACC/AHA/Multisociety Dyslipidemia Guideline, the European Atherosclerosis Society, and the National Lipid Association now all recommend universal testing in adults at least once.

The reasons most executives haven't been tested are simple: NHS GPs don't routinely order it, standard occupational health screens don't include it, and most private "executive medicals" still rely on outdated lipid panels. You have to ask for it specifically — or work with someone who knows to look.

How to Read Your Lp(a) Number

Lp(a) is reported in either nmol/L (the more accurate unit) or mg/dL. Always insist on nmol/L. The rough interpretation for men over 40:

  • Under 75 nmol/L — low risk. You can essentially ignore this one variable and focus on the rest of your cardiovascular picture.
  • 75–125 nmol/L — borderline. Tightens the case for aggressive ApoB control.
  • 125–250 nmol/L — high. Roughly 40% higher lifetime ASCVD risk. You need a serious cardiovascular protocol.
  • Above 250 nmol/L — very high. Risk is approximately doubled. This is the band where every other modifiable lever matters more, not less.

About one in five men over 40 will land in the high or very high band. If you're in that group, the message isn't panic — it's that the rest of your cardiovascular hygiene now has to be excellent rather than average.

The Executive Lp(a) Protocol

The single highest-leverage move is to drive ApoB lower than the standard targets. That typically means a statin, often paired with ezetimibe, sometimes with a PCSK9 inhibitor — clinically directed, not self-prescribed.

Get a CAC score. Lp(a) tells you genetic risk; CAC tells you what that risk has already done to your arteries. Together they make the case for how aggressive your treatment needs to be.

Lifestyle Levers That Still Matter

You cannot meaningfully lower Lp(a) by living well, but you can meaningfully lower your overall cardiovascular event risk. Three levers carry the most weight for executives:

  • Eliminate visceral fat. It is the upstream driver of insulin resistance, inflammation, and ApoB elevation.
  • Build cardiovascular fitness with zone 2 cardio. VO2 max is the single most powerful longevity marker and a partial buffer against the arterial damage Lp(a) accelerates.
  • Stop nicotine in any form. Smoking and Lp(a) are a particularly destructive pairing.

Blood pressure, sleep, and alcohol all sit in the second tier — important, but secondary to the three above.

Emerging Therapies — Pelacarsen and Olpasiran

The first generation of Lp(a)-specific drugs is finally arriving. Pelacarsen, an antisense oligonucleotide, is in the phase 3 Lp(a)HORIZON trial with cardiovascular outcomes data expected in 2026. Olpasiran, an siRNA therapy, has produced sustained Lp(a) reductions of up to 100% in earlier-phase studies.

These will not be over-the-counter solutions. They will be prescribed agents reserved initially for those with both elevated Lp(a) and established or very high-risk cardiovascular disease. But for executives in their forties testing now, this is exactly the window where knowing your number positions you to access these therapies the moment they become clinically available.

Lp(a), CAC, and ApoB — How They Fit Together

If you take one mental model away from this article, take this one: ApoB tells you how many atherogenic particles are circulating right now, Lp(a) tells you how dangerous each of those particles is genetically, and CAC tells you what damage has already been done.

You need all three. An executive who has only ever had an LDL and total cholesterol checked is flying blind on the most important variable in their long-term health. The investment to fix this is one blood test, one scan, and a conversation with someone who knows what to do with the numbers.

Can lifestyle lower Lp(a)?

Marginally and inconsistently. Some research suggests aggressive weight loss can move Lp(a) by 10–15%, but the change is small relative to genetic load. Do not rely on lifestyle to fix high Lp(a) — rely on it to lower every other risk factor that compounds with it.

Should I take niacin for high Lp(a)?

Niacin lowers Lp(a) modestly (around 20–30%) but the cardiovascular outcomes trials have been disappointing, and the side effect profile is not benign. Most preventive cardiologists no longer recommend it as a primary strategy. Your money is better spent on getting ApoB to target.

Should my children be tested?

Does TRT affect Lp(a)?
