Your energy, your drive, your morning erection, your patience with the board — those say something different. The disconnect isn't in your head. It's in which number you measured. Why the standard testosterone panel quietly lies after 40

Most GPs order one number: total testosterone. They compare it to a reference range built from a population that includes 19-year-olds and 75-year-olds. As long as you fall somewhere in the middle, you get told you're "in range." That's the lie.

After 40, three things happen at once. Total testosterone declines slowly. Sex Hormone Binding Globulin (SHBG) rises sharply. And the usable fraction of your testosterone — the free testosterone your tissues can actually access — falls off a cliff. A man with total T of 580 ng/dL and SHBG of 60 nmol/L can have the symptomatic free T of a 70-year-old. The standard panel calls him normal. His body knows otherwise.

This is why nearly every executive over 40 I've coached has been told he's fine on paper while sitting across from me describing the exact symptoms of clinical hypogonadism. The wrong test was ordered.

What total testosterone actually measures

Total testosterone is the entire pool of testosterone in your bloodstream — about 98% of it bound to carrier proteins and biologically inert, plus the small free fraction. Roughly 44% is locked to SHBG (unavailable to your tissues), about 54% is loosely bound to albumin (technically accessible but slow to release), and 1–3% is free.

Think of it as the size of the warehouse, not the inventory on the shelves. A big warehouse doesn't help if everything inside is bolted to the floor. Total T is useful as a starting point and for tracking trends, but on its own it tells you almost nothing about what your body can actually use.

What free testosterone actually measures (and why it's the number that matters)

Free testosterone is the unbound, biologically active fraction. It's the only testosterone that can cross cell membranes, dock at androgen receptors, and produce the downstream effects you actually care about: drive, muscle synthesis, cognitive sharpness, libido, recovery, confidence under load.

The clinical data is unambiguous. A 2016 study in the Journal of Clinical Endocrinology & Metabolism found that men with low free testosterone and normal total testosterone still showed every symptom of hypogonadism — sexual dysfunction, poor general health, declining physical function. A 2024 PMC review found that calculated free testosterone correlates more strongly with lean mass, muscle strength, and physical performance in older men than total testosterone does.

In other words: if you only know your total T, you don't know what's happening in your body. You know what's happening in your bloodstream.

The SHBG problem — the silent thief of free testosterone after 40

SHBG is the protein that grabs testosterone and won't let go. The more SHBG you produce, the less of your total T is actually bioavailable. And SHBG rises with age — by 60, many men have SHBG levels 40–50% higher than they did in their twenties.

For executives, the SHBG story is worse than the baseline age curve. Several drivers of high SHBG are also signatures of the executive lifestyle: chronic alcohol intake, low calorie/low-carb dieting cycles, hyperthyroid drift, low body fat combined with insufficient food, and certain medications. Conversely, very low SHBG is associated with metabolic syndrome and visceral fat — the CEO belly — and signals a different but equally serious problem.

This is why two men with identical total testosterone can have wildly different lives. The man with SHBG of 25 has plenty of free T. The man with SHBG of 65 is symptomatic. Without SHBG on the panel, neither of them knows which one he is.

Free testosterone vs total testosterone: the executive numbers to track

Here's what to actually measure, and where the numbers should sit for a 40+ executive who wants to perform, not merely scrape into the lab's reference range:

Total testosterone: 600–900 ng/dL (20.8–31.2 nmol/L). The lab will call 300+ normal. Don't accept it.

SHBG: 20–45 nmol/L. Above 50, free T is at risk. Below 20, suspect metabolic dysfunction.

Free testosterone (calculated, Vermeulen equation): Above 0.30 nmol/L (≈8.7 ng/dL) for symptom relief. Optimal performance territory sits 0.35–0.50 nmol/L. British Society for Sexual Medicine flags below 0.225 nmol/L as a trial-of-TRT threshold — that's the floor, not the target.

Albumin: 4.0–5.0 g/dL. Usually fine, but required to calculate free T accurately.

Free T is the biologically meaningful number. Total T and SHBG are the inputs that let you understand it. Order all three.

How to get tested properly (and avoid the wrong panel)

Three rules.

First, time the draw correctly. Testosterone peaks between 7:00 and 10:00 AM. A draw at 2:00 PM can read 20–25% lower than the same man two hours after waking. Always test before 10:00 AM, fasted if possible.

Second, demand the right markers on the form. Total testosterone, SHBG, albumin, and either a calculated free testosterone (Vermeulen equation) or a free T measured by equilibrium dialysis. Do not accept "free testosterone direct" assays — they're unreliable. The calculated value from Total T + SHBG + Albumin is the clinical standard, and it correlates with gold-standard equilibrium dialysis at r = 0.92.

Third, retest in the same window. If you're tracking a change — protocol, supplement, sleep intervention — repeat the draw same lab, same morning window, same fasting state. Otherwise you're measuring noise.

Add LH, FSH, oestradiol (sensitive assay), prolactin, and a full thyroid panel if this is your first hormone workup. They'll tell you why free T is where it is, not just that it's there.

The 5-lever protocol to raise free testosterone naturally

Most men over 40 don't need TRT. They need the four or five levers they've never properly pulled. In order of leverage:

1. Fix sleep — properly. Roughly 70% of daily testosterone is released during deep sleep. If you're sleeping 6 hours, snoring, or waking unrefreshed, you're bleeding T every night. Screen yourself for obstructive sleep apnea — 30% of men in their 40s have it, 80% don't know — and read why poor sleep is destroying your testosterone after 40. Nothing else moves the needle if this is broken.

2. Drop visceral fat. Belly fat aromatises testosterone into oestrogen and drives the SHBG pattern in both directions depending on phenotype. Losing 5–8 kg of visceral fat is the single biggest hormonal intervention available to most 40+ executives.

3. Audit alcohol. Even moderate drinking suppresses T, spikes cortisol, and trashes the deep sleep your hormones depend on. Read alcohol and testosterone in men over 40 for the exact mechanism. Three drinks a night isn't a hobby — it's a hormonal tax.

4. Lock in the foundational nutrients. Vitamin D, zinc, and magnesium are the three most commonly deficient nutrients in 40+ males, and each is rate-limiting for testosterone production. Test, dose to repletion, retest. No guessing.

If free T stays below 0.225 nmol/L after three to six months of disciplined work — sleep fixed, body composition corrected, alcohol controlled, nutrients repleted, cortisol managed — and your symptoms persist, a properly supervised TRT discussion is the rational next step. Not a moral failure. Not a shortcut. A medical decision based on the right data.

This is the part where the panel matters most. Going on TRT with only total T to guide you is malpractice. Going on TRT with total T, free T, SHBG, oestradiol, LH and a hematocrit baseline is medicine.
