What Homocysteine Actually Is (And Why It Matters at 40)

Homocysteine is an amino acid your body produces every time it metabolises methionine — the protein-derived building block that drives methylation, the chemical process that repairs DNA, regulates gene expression, makes neurotransmitters, and detoxifies hormones. Homocysteine is supposed to be recycled fast. Folate and vitamin B12 convert it back to methionine. Vitamin B6 sends it down a second pathway into glutathione, your body's master antioxidant.

When that machinery works, homocysteine stays low. Lab reference ranges are misleading. Most UK labs will flag homocysteine as abnormal only above 15 µmol/L. The evidence has moved well below that.

Two reasons that matters. First, the people who most need it — high-performing men who already eat well, lift, and feel sharp — are exactly the people who get reassuring "normal" results from the markers that are checked. Homocysteine catches the silent metabolic and methylation friction those markers miss. Second, the intervention is cheap, safe, and fast. There is almost no reason not to know your number.

What Drives Homocysteine Up

B Vitamin Deficiencies

The remethylation pathway needs folate and B12. The transsulfuration pathway needs B6. Run short on any of the three and homocysteine accumulates. Vitamin B12 in particular is widely under-replete in men over 40 because absorption declines with age and is further impaired by metformin, proton pump inhibitors (PPIs), H2 blockers, and chronic alcohol use. A "normal" serum B12 above 200 pg/mL does not mean functional sufficiency — many men with serum B12 in the 200–400 range have elevated methylmalonic acid, the more sensitive marker.

The MTHFR Wildcard

The MTHFR gene codes for the enzyme that converts folate into its active form, methylfolate. Roughly 40% of the population carries one copy of the C677T variant, around 10–15% carry two copies (homozygous), and 10–15% more carry the A1298C variant. Homozygous MTHFR C677T can reduce enzyme activity by up to 70%, which means you cannot efficiently methylate the folic acid in fortified foods and standard multivitamins, and your homocysteine runs higher by default.
